Visiting Nurse leaders, Natasha Fowlks, Executive Director Hospice Atlanta & Palliative Care, and Beverly Roberts, LPN, Palliative Care leader, slated as panelists for the Understanding Dementia in the African American Community Forum Saturday, September 28th. Their expertise and insights on home-based care giving solutions provided valuable information and support to all attendees.
As part of the GUIDE model, this vital panel conversation aims to shed light on the unique challenges and opportunities faced by African American individuals and families dealing with dementia. The GUIDE Model (Guiding an Improved Dementia Experience) is a new voluntary, nationwide model for Medicare Part B providers from the Centers for Medicare & Medicaid Services (CMS) that aims to improve the quality of life for people living with dementia, reduce strain on unpaid caregivers, and help people remain in their homes and communities through a package of care coordination and management, caregiver education and support, and respite services.
