About Visiting Nurse’s Service Options Using Resources in Community Environments (SOURCE) Program

Service Options Using Resources in Community Environments (SOURCE), creates a continuing care network for at risk individuals who need medical attention, supportive assistance and prefer to stay in their own homes and communities. This alternative to nursing home placement provides comprehensive, preventative medical care along with in-home support services. Care management is central to identifying and providing appropriate service. SOURCE service options include the following:

Comprehensive Primary Medical Care

  • Specialist care coordinated through the participant’s primary care provider
  • Visiting Nurse Care Manager to coordinate care

Adult Day Programs

  • Daytime care and supervision in an adult daycare center
  • Available up to five days a week
  • Therapeutic activities and outings
  • Meals adapted to prescribed diets
  • Dementia-specific care

Personal Support Services

  • Assistance with meal preparation, hygiene and nutrition
  • Light housekeeping, shopping and other support services

Respite Care

  • In-home & out of home (where available)

Home Healthcare Services

  • Skilled nursing with medication management
  • Medical care management

Emergency Response Services

  • 24 hour, 7 day in-home electronic support
  • Personal Emergency Response System

Home Delivered Meals

  • Daily or weekly delivery of nutritious meals
  • Modified meals to meet special nutritional needs

Transportation

  • Coordinated for medical, dental, vision services

Enhanced Case Management

  • All social and medical needs are supported by individual Care Managers

Alternative Living Services

  • Alternative residence for persons unable to remain independent in their homes
  • Meals, personal care and supervision

SOURCE Eligibility Requirements

  • Have SSI Medicaid or Public Law Medicaid coverage
  • Functional impairment caused by physical limitation
    NOTE: Alzheimer’s and dementia are physical conditions
  • Unmet need for care
  • Approval of care plan by applicant/client’s physician
  • Services fall within the average annual cost of Medicaid reimbursed care provided in a nursing facility
  • Approval of an intermediate level of care (LOC) certification for nursing home placement
  • Client chooses community based rather than institutional services
  • Health and safety needs can be met by SOURCE
  • Participation in no other Medicaid waiver program
  • Medicare home health services do not meet their needs
  • The need for services beyond home delivered meals
  • A home environment free of illegal behavior and threats of bodily harm to other persons
  • A client is NOT required to be homebound to receive CCSP services

Our Visiting Nurse Health System and Hospice Atlanta Home Health and Hospice teams were proud to be part of the Aging Better Expo, hosted by our valued healthcare partner, Kaiser Permanente.
The event brought seniors and community partners together for a day focused on health, wellness, resources and connections that help older adults live and age well in their homes!
Great event connecting one-on-one with seniors, answering questions, sharing information about our full care continuum filled with services to help seniors age in place.

Grateful for strong community partnerships that help build strong nonprofit leadership.
For the past five years, OnBoard has been an important pipeline for recruiting talented, committed leaders to the Visiting Nurse Health System | Hospice Atlanta Board of Directors. Their work has helped us connect with individuals who bring fresh perspectives, valuable expertise, and a genuine commitment to our mission.
We’ve seen that impact firsthand. Mel Coker, now in her first term as Board Chair, came to our board through OnBoard and stepped into the chair role following Nicole Hart’s successful completion of her term. And now, we are excited to welcome Amy Katnik as the newest leader joining our board through the OnBoard pipeline.
