Wound Care Program

Our Wound Care Program provides specialized care in the comfort and safety of the patient’s home. The program is designed for patients recovering from surgery, managing a chronic or complex wound, or requiring ongoing ostomy or continence support. Visiting Nurse Health System’s goal is to promote healing, prevent complications, reduce avoidable hospitalizations, and help patients regain independence.

The program includes clinical oversight by a certified Wound, Ostomy and Continence Nurse (WOCN). The WOCN collaborates with the patient’s physician or authorized provider and the home health care team to guide evidence-based treatment, evaluate healing progress, recommend changes to the plan of care, and provide education to patients, caregivers, and clinicians.

Highly skilled Visiting Nurse professionals provide expert wound assessment and treatment with compassion, understanding, and respect. We help patients and caregivers gain the knowledge and confidence needed to safely manage wound care between visits while relieving some of the burden placed on families.

Home health wound care requires an order from a physician or authorized provider and is covered by Medicare and many insurance plans when eligibility requirements are met. Services are provided according to the patient’s individualized plan of care and may include:

  • Comprehensive wound assessments and measurements
  • Individualized wound treatment plans
  • WOCN consultation and clinical oversight
  • Coordination with physicians, surgeons, wound clinics, and other providers
  • Negative-pressure wound therapy management
  • Advanced dressing selection and application
  • Ostomy assessment, education, and support
  • Pressure-injury prevention and treatment
  • Infection monitoring and early identification of complications
  • Pain and symptom-management education
  • Nutritional education to support wound healing
  • Patient and caregiver training
  • Ongoing monitoring of wound-healing progress
