About Community Care Services Program (CCSP)
Community Care Services Program (CCSP), helps senior adults and people with functional and/or cognitive impairments stay in their homes and communities for as long as possible. CCSP is a program of the Georgia Department of Community Health. Visiting Nurse Health System provides case management oversight for over 2,000 CCSP clients in the Greater Atlanta area at any given time allowing them to access community-based care and retain their independence.
