About Community Care Services Program (CCSP)

Community Care Services Program (CCSP), helps senior adults and people with functional and/or cognitive impairments stay in their homes and communities for as long as possible. CCSP is a program of the Georgia Department of Community Health. Visiting Nurse Health System provides case management oversight for over 2,000 CCSP clients in the Greater Atlanta area at any given time allowing them to access community-based care and retain their independence. The CCSP service options include the following:

Adult Day Program

Available up to five days a week

  • Daytime care and supervision in an adult daycare center
  • Therapeutic activities and outings
  • Meals adapted to prescribed diets
  • Dementia-specific care

Alternative Living Services

  • Alternative residence for persons unable to remain independent in their homes
  • Meals, medically-oriented personal care and nursing supervision

Emergency Response Services

  • 24 hour, 7 day in-home electronic support between the client and a medical control center
  • Personal Emergency Response System

Home Healthcare Services

  • Skilled nursing services—physical, speech & occupational therapy, home health aide, medical social services and medication management
  • Medical case management

Respite Care

  • In-home & out of home (where available)

Community Care Unit!

Personal Support Services

  • Assistance with meal preparation, feeding in addition to hygiene support like bathing, toileting and dressing
  • Light housekeeping, shopping and other support services

Transportation

  • Senior transportation services coordinated for medical, dental, vision appointments
  • Wheelchair accessible transportation options available for senior adults and people with functional and/or cognitive impairments

Home Delivered Meals

  • Daily or weekly delivery of nutritious meals
  • Modified meals to meet special nutritional needs

Case Management

  • All social and medical needs are supported by individual
    Case Managers

Community Care Services Program (CCSP) Eligibility Requirements

  • Functional impairment caused by physical limitation
    NOTE: Alzheimer’s and dementia are physical conditions
  • Unmet need for care
  • Approval of care plan by applicant/client’s physician
  • Services fall within the average annual cost of Medicaid reimbursed care provided in a nursing facility
  • Approval of an intermediate Level of Care (LOC) certification for nursing home placement
  • Medicaid eligible after admission to CCSP client chooses community based rather than institutional services
  • Health and safety needs can be met by CCSP
  • Participation in no other Medicaid waiver program
  • Medicare home health services do not meet their needs
  • The need for services beyond home delivered meals
  • A home environment free of illegal behavior and threats of bodily harm to other persons
  • A client is NOT required to be homebound to receive CCSP services

We've had a great day honoring our Veteran hospice patients for their courage, service, and connection. Our Veteran Volunteer retired Navy Captain Alan, traveled around to our patients’ homes today, honoring their service with a We Honor Veterans pinning event. A few of our patients honored today are shown here: Duane, an Air Force pilot, and later a commercial pilot, with a flawless 30-year flying record, and Edward “Ted,” a Navy veteran and centenarian honored at age 102. 
Alan, our Volunteer and a retired Navy Captain who served aboard the USS Long Beach during the Vietnam War, has spent over 15 years giving back to fellow veterans through visits and veteran pinning ceremonies. Through these events, Alan brings honor, understanding, recognition, and the unique bond that only a fellow service member can share.
