From physical therapy to skilled nursing during recovery, and from remote vital sign monitoring to long-term care at home or Hospice Atlanta Center, Visiting Nurse helps patients reduce emergency room visits, re-hospitalizations and nursing home placements.

Who We Serve
Visiting Nurse helps patients meet their goals of care through specialized programs and a focus on the priorities of the individual. These services range from remote technology-supported care with telemonitoring, to care addressing the unique needs of Veterans and their loved ones. Disease management programs for patients living with life-limiting illnesses, including COPD, cancer, and heart disease, provide supportive care and caregiver education to ensure the best outcomes.
Visiting Nurse is the community safety net for care at home. While many of the services we provide are covered by Medicare, Medicaid and other forms of health insurance, we rely on the generosity of our donors to bridge the more than $3 million gap we spend on patient care for the underinsured.
