Our Certified HeartCare at Home™ Program
Our Certified American Heart Association (AHA) Home Health Heart Failure Program helps our patients safely transition from hospital to home. Our skilled clinical team teaches patients and caregivers how to manage health conditions with confidence, supporting the road to recovery and helping prevent hospital readmissions.
This certification assures our patients that Visiting Nurse Health System’s home health’s heart failure program is recognized by the American Heart Association, based on professional evaluation criteria designed by experts in heart failure and home health.
Our HeartCare At Home Program™ includes:
- LVAD trained Registered Nurses
- Telehealth monitoring to detect early signs of complications
- Specialized Inotropic Therapy management
- Heart failure symptom management such as fatigue, weakness, rapid or irregular heartbeat and fluid overload
- Home-based care for patients with advanced heart failure
- Early intervention to reduce hospital readmissions
Tips for managing heart failure:
- Keep records of your weight. Sudden or steady weight gain could mean you are retaining fluids.
- Eat healthy, low-sodium foods. Too much sodium makes your body retain fluids. In general, limit sodium to 500 mg per meal.
- Get regular physical activity. This will improve your blood pressure, help manage high blood pressure and diabetes, and help you maintain your independence and quality of life.
- Take medications as prescribed.
- Do not smoke, drink alcohol or take illegal drugs.
